If you are of indian origin having relatives in india. Then not to worry your relatives will take care to protect you from problems you are most likely to face.
If you are of foreign origin, The first important matter to take care of is water & where you eat what you eat.
Avoid eating from roadside stalls.
Even the goodlooking hotels restaurants may have dirty kitchens inside.
Always buy bottled water from a good reliable shop ( well this is easier said than done) be careful from where you buy your bottled water, Counterfeits of reputed brands are most common in india,
Always eats hot freshly prepared food. If bottled water is unavailable, You can try hot tea or coffee, Or make your own arrangements to boil water.
Always remember that waterborne diseases and food poisoning is very very common problem faced by the foreign visitors in India.

Go to the CDC web site. Most doctors do not know exactly what is needed in the month you are going. It is not just one or two shots. You may have to get multiple ones spread out 4-6 weeks apart. So, plan well.
Also, plan everything about the city you are going to.
Finally, take some of your favorite non-perishable item with you (cheese, cereal etc), esp. if you have room in your bags.
Good to take the sanitary hand-cleaner since you will need it as you move around in India.
Do NOT eat foods that are not hot when served. Do NOT drink any form of milk, yogurt, sweets, and non-bottled water. Do NOT trust bottled water from any store.
Take 2 bottles of the strong Anti-Diarrehea medications with you. I would even recommend your favorite paper-tissues (facial and toilet) if you have room in your bag.
Take TWO 110V to 220V converter, but more importantly take 2 surge protectors. I blew two of them in my trip to India in 2006, and everyone knows you need good surge protection!
Lastly, if you are taking your laptop, do not assume internet connection from anywhere/everywhere. It is a real pain to get 'true' high speed connections depending on which parts you are going to.

OK take all the necessary shots (embassy will telll you you dont need them but dont believe them) Ask you GP
Take anti diahreoa tablets.
Avoid eating from open food stalls. Eat only hot food and not leftovers.
Take you own cereals and milk powder. Dont drink Indian milk.
You'll do better as a vegetarian ie dont eat meat.
Eat only fruit that you need to peel eg banana, coconut, mandarins etc.
Drink only boiled ot bottled water. Some bottled water are not save either so best to boil it.
All the above are easy to take care of and its not a real burden. After that just enjoy your trip.

which parts of india? can you speak their language? (either tamil or hindu) (don't worry, the airport personnel can speak english)
India is a nice place, but be aware of the locals, my senior once said, the theft there are highly expert, so watch out for your item.
There will be beggers, if you give them money, more begger will come to you.
There will be trickster, so watch out who come to you and don't go with them. The people may not be well manner but just be nice to them. And be careful what you are spending.
My case is I went to buy a bottle of orange juice, but the bottle and the liquid seen to be tampered with, that's not the worst, the market price I found out is 10 rupee but that guy sold me 50.
The hotel servants need to be tipped when helped, but i forget
The tap water cannot be drink.
I once visited central india at 2005 at a place called Babina. It's nice place.

Just be happy there is a lot of places in INDIA here you can go just try to move towards the historical places of INDIA

Try to know the Indian culture. Prefer Indian foods and have smile at all the time. Take care of water and your dress code. If you follow this then you can feel being at your home.

you need no preparations... treat India like any other country... do not underestimate India... you would be amazed once you land in this country... you will get every thing what you do in US...

You need to get some shots for Malaria and other such diseases. Ask your doctor about it.
