I presume we are talking about a PC here.
If you are unable to access/ alter programs and system settings then there is little you can do other than reinstall your system.
This is not for amateurs to attempt!
STEP1 back up everything you possible can including all settings and passwords that you can remember/access.
pay particular attenti0on to your valuable data. reinstallation of windows means formatting your hard drive. you will lose everything that you do not have a copy of on removable storage !!!!!!!!!!!!!!!!!
STEP2 Reinstall the operating system and any additional components - make sure you record all entries you make for system/program settings/passwords
STEP3 Copy back your rescued data.
STEP4 Type out all your setting details and passwords in plain unencrypted english in your word processor. Now copy onto a floppy or CD. safely store this in a place not near your computer. Delete and overwrite the file on your computer.
STEP5 setup your calender to remind you not to be so foolish ever again every time you switch your machine on.

If you have access to or know someone who has ERD commander, this will be the answer to all your problems. You boot off the CD to its version of windows, select locksmith, choose the username that you need to change and then enter a new password. Reboot and you are done. Easy as that. The other way is to boot to safe mode and then ente administrator as the username. This one is normally not password protected if it was installed at the factory. If you changed it later on then you are out of luck using this method The third method would be to boot off Knoppix cd, and replace the SAM file (this is where all the passwords are stored in windows XP) with the SAM file that contains a username and password that you know. The other method would be to install a parallel version of windows, leaving the first one intact. This will give you access to all the files and someone told me that if you then transferred the registry of the old install to the new one, you might get all your installed softwares back. Don't hold me to this one. I have not tried it myself. Like I said, ERD Commander. Works like a charm.

if you have a windows cd, let the computer boot off the cd, then select repair my os, when it gets to the in stall screen saying install drivers in the bottom lefthand corner, press crtl & f10, it will then bring up the password console you can then reset the user account,

easy!
just reboot in safe mode (restart and keep hitting f8) u'll find there's another acc (it's by default) named 'administrator'
use it to remove your passwords
