Here is a good prank that requires a little mechanical expertise but won't hurt him or the car. I call this the butt remover because it quickly removes the person butt from it's seat. We did this to several people in highschool and I laughed every time.
Here's what you do. 1. Get about 10' of speaker wire (2 copper wires in seperate insulation) the smaller the better. 2. attack one end of the wires to the positive and negative terminals of the starter solenoid in the engine compartment of his car. 3. reun the other end of the wire along the wire already in there all the way back to the firewall, shove it through then run it under the floor mat and seat. 4. Now put the ends that you ran into the cab of the car up through the crease in the driver seat. put the wire tips abouta half inch or so apart (DO NOT LET THEM TOUCH). 5. When your pal starts the engine a small amount of electricity will go to his *** making him jump out of his seat (without the car being started yet), best part is they usually start the car again and get zapped again before they figure out someone is playing a trick on em. Have fun with this one and don't forget to help remove the wire after you get your laugh.

Im not sure, about the radiator but stuffing anything into any part of a car's inner works risks damaging it, especially the fuel tank or exhaust pipe. There are better things to do however that may prove even more devious and less damaging. If you have access to his car or underwear drawer, why not hide something in his room or car that over time will cause trouble. For example, stick some frozen fish in between the area between the hubcaps and his tires (making sure not to damage the brakes or anything there) After awhile when it thaws he'll begin to smell that horrible fishy smell, but his initial instict will be to check the cars interior, trunk and under the hood. Go nuts laughing behind his back and taunting him for as long as it takes him to figure out what's really going on.

Super Glue The American Flag to it? Not sure if its breaking any laws. Or just stick the flag down using the radio antennae. Then stick a Union 76 gas station ball on top it. (as a stopper).
Super glue his undies to the back windshield or lower right bumper. He'll never notice if he just gets in and drives off. Unless he is the supercareful person who does the thourough walk around the car to find any objects, such as toys behind his car or tires.

Forget messing with a car in the manner you were suggesting. Those things could become deadly pranks. You mentioned underware... find some itching powder and put some in his shorts, something like that. Even could saran his car.

Tape a big balloon to his exhaust pipe. Then put the underwear on his windshield. He'll think "Ha ha. Those guys." Then he starts his car thinking "I'm safe from any harm." Then the balloon blows and he gets frightened.

You shouldn't do anything which would have legal, financial or criminal repercussions. I would recommend you avoid messing with his car. If you have to do something, if this is a source of humor for you for some reason, how about a phone call pretending to be a government official.

Why don't you try filling his car with inflated balloons. We did this to a friend's hotel room at a motorcycle function one year, then we waited until he headed back to his room after a few too many cocktails, and he had to go to the bathroom real bad. It was hilarious watching him try to get to the bathroom through hundreds of balloons.

These sound like pranks that can go wrong. Try placing saran wrap under the seat, over the bowl of the commode.
Be careful of any prank it can backfire.
