After physical death, you pass through an event horizon some describe as going through light where your two polarities combine. Some don't choose to do so and hang around in the ether around us.
Some newly physically deceased don't even realize they are "dead" for quite some time and just hang around. If their exit from this world was particularly traumatic, they'll hang around until justice is finally done also.

I think ghosts appear when people in the present look back on an event in the past, The 'ghosts' aren't dying over and over again, but we're viewing it like a rerun on television. For them, it's only just now occuring, so they don't know they've died. Maybe UFO's are looking into the future.
What happens if you break the speed of light? It could put a wrinkle or fold into the space-time continuum, and events that are not in chronological order are then placed next to each other.

if someone dies tragically and suddenly, the suddeness of the death causes them to not realize they are dead! then they will continue to replay the events until either they can be excercised or the world ends. the other ghosts who know they are dead will sometimes try to make themselves known to the people around it. then its a haunting. they can also be forced to leave by excerciseing them. then there are poltergiests and you don't want to deal with them!! that taks some powerful prayer to get rid of them!!

some being know htey have passes other do not. Usually those who have not passed cause, what is known as "residual Hauntings" Basically they are doing what they did before they died. Otehrs who usually play with us, know they are dead but they do not want to move on for fear of where they may go or fear of leaving everything behind.

a violent death brings them back to the point that we can see them, such as a car crash or a blade to the main artery, and no they dont know they are no longer living, the continue their life as if nothing happened, in their own world.
