Blood is absolutely not 'formed in the heart'- the heart pumps the blood around the body. Blood cells are produced by the bone marrow. The fluid components of the blood are mostly water and plasma proteins which are produced by various cells (e.g. fibrinogen for clotting is produced in particular cells in the liver).

Blood production rate is going to vary based on need (e.g. if you lose blood, your body will need to make more faster). Red blood cells are actually recycled in the body.

It is formed in the marrow of long bones specifically and the pelvic bones that why breaking your hip is so dangerous for the elderly. All your heart does is pump the blood thru the system. The basic replacement rate varies according to each person and the current health of the body.....an average person can only give 1pint of blood every 6 wks as a donor....but can donate platelets only much sooner. So platelet production can keep up but not necessarily volume production.

I hope you imply the formation of red blood cells in the adult.
Well, it is formed in the red bone marrow. About two million RBC are made per second.
There are 5 million RBC per cubic mm of blood. In the same volume there are about 12000 WBC, and half million blood platelets. They are all made in the bone marrow. For WBC the forming tissue is called leucopoietic tissue and the RBC forming tissue is called erythropoietic tissue. Platelets are formed by megakaryocytes of the bone marrow.
