Firstly, find out what the job is about as this will help you tailor the reference.

To start is quite simple: Dear Mr Smith / Sir

I have been asked to supply a reference for X whom I understand is seeking employment with your company as a xxxxxx.

I have known X for Y years as a friend.....

Employers are interested in several things, the first of which is honesty. A good phrase to use is "X is a woman of complete honesty. Thoroughly trustworthy, she can be relied upon to act with integrity in all matters".

What about her interpersonal skills? "X is a very friendly, outgoing person who works well both on her own and as part of a team".

And a final flourish "I can recommend X to you very highly. I am sure that she would be an asset to you company".

Good luck - oh, and remember a) to spell check it and b) Dear Sir gets Yours faithfully, Dear Mr Smith gets Yours sincerely. I always have to remind myself of that one!

A few useful phases:

I have never caught her being dishonest, in any case it was negligent of me for not locking the petty cash tin.
I have no evidence that her frequent trips to the seedy part of town have any immoral purposes.
She must be very family orientated in view of the amount of time she takes off work to attend family funerals.
Her attendance at the AA meetings is undoubtedly to do with a research project that she is undertaking.

Something like this:
Your address at the top, leave a couple of lines and put the name and address to where you are sending it. then leave a couple of lines and put the date. leave a couple of lines and then:

Dear Sir/Madam

I have known [FULL NAME] for .... years and have always found him/her to be honest, reliable and considerate and have no hesitation in providing this character reference.

Yours faithfully
(sign here)

YOUR NAME
+++
it would have helped it you'd said what it was for and I could have been more specific but hopefuly you'll get the idea now.
