Call 800-937-8997, then say english. When it askes for your phone numbers, just say "represenative" this will get you to someone immediatly. Then tell them you will need your PUK code. It will be ready to use immediatly. It will ask you to change your password. The default code for the 6030 is 12345. If you set it to this and you have problems later you can stop by your local T-Mobile corporate store and they will help you.

call your service provider. My friend had this problem and couldn't get anything to do anything until she entered a PUK code. So, she called Cingular and they did something and she got the code and her phone was just as new!

PUK (Personal Unblocking Key) codes are supplied with your SIM (Subbscriber Identity Module) card by your service provider. Please contact your service provider for getting the PUK code.

I think it's on the box that the phone comes in. If you've still got your box, have a look on it. Otherwise contact your service provider.
