For the most part, media is responsible. Reporters sent in foreign countries need to send as stimulating reports as possible to their editors back home. Otherwise, their articles won't be accepted and published.

Even though they may be aware that they are more or less biased, readers are always looking for new and exciting articles. Media seems to have become so commercialized that they have forgotten their true mission. They are just selling news like car dealers. This is particularly true with U.S. media.

Readers are easily "brainwashed" by those false information about foreign countries even in this Internet age. When readers have personal or collective grudge against the country in question, biased and malicious information is so effective to brainwash them overnight.

Propaganda is spread by the media and the govt. It's hard for a country to go bomb the crap out of another one if they let their populace see the other country as being full of people just like themselves. So, they put forth propaganda to make the other people look evil, in order to rally the peasantry.

Every country is guilty of it.
