A pre-op transsexual has (through hormones) the body of the gender they identify with, except for the genitalia. A post-op transsexual has (through surgery) the body and genitalia of the gender they identify with.

I am a pre-op transsexuals even though I think of myself as 100% female. Hopefully I will be a post-op transsexual with in the next year.

'Pre-op' just stands for 'pre-operative', meaning before any surgical operation to reassign the sex. 'Post-op' stands for 'post-operative', meaning after such surgical operations. There is also 'non-op', meaning 'non-operative', for transsexuals who, for whatever reason, choose not to surgically alter their bodies (it may be for lack of money, not minding having male genitals, etc., etc.). Neither type is 'better' than any others (I've seen transpeople scorn non-ops and shun pre-ops, sadly), they are merely on different stages of their transition.
