When I say we, I mean we as in all space programs in all the countries, not just the U.S.'s NASA.

Heavens no.

We are only now at the edge of our own solar system. The Voyager space craft are only a few short light hours from Earth. The nearest suspected black hole, that I am aware of, is at the center of the Milky Way galaxy.

The Milky Way Galaxy is about 100,000 light years in diameter. Since we are near the edge of the galactic disk the center of the galaxy is 45,000 to 50,000 light years away. And our deepest penetration of space so far can be measured in light hours, not light years.

So the short answer to your question is no. We, the human race, have not lost a space craft or anything else to a black hole.

Finding a real Black body in the universe is yet to be accomplished. Black hole is an approximation made according to a part of Einstein's proof of E=mc^2. Humans started finding for a real presence of Black body. We were/are successful in approximating by visualizing them through Radio telescopes. It is interesting fact to know that the nearest black-body to us in the only one that is present in the center of our galaxy Milky way...which is no less than TRILLION Miles away from us.

May be someday....in near future(in a span of 500 Yrs) we will be able to reach a real Black body and start experimenting on it! ;)

The two Voyager spacecraft have not yet left the solar system. They are the human artefacts furthest from this planet. As they haven't yet left the solar system, they certainly have not left the galaxy or been anywhere near a black hole.

The closest any of our space probes have got to a black hole is several thousand light years.

Just my opinion, but I don't think we've ever been able to send anything even remotely close to a black hole.

possibly, NASA has sent two space probes out of our galaxy, but lost contact with them, it is possible that they will end up in a black hole

I heard on the news recently, that we did have a probe that was lost into a black hole..so I say yes we did.
