1) Who will launch the first attack?
2) and against which nation?

As i see it is enevitable that there will be a nuclear war at some point. Mankind has never created a weapon and then failed to use it.

I think Iran will probably be the first hit. Hopefully, because like you I belief that a Nuclear Attack is inevitable. Better them than us. Because realy, if it isn't them, then it absolutely will be us. We may be able to stop 99 out of 100. But it only takes that one.

But if it is Iran that is hit first, whether by the US, Israel, Great Britain, or any other Nato/Allied Country, it will be a weaker nuke, detonated in a bunker buster or similar weapon, underground. The Nuclear Fall out would then be minimized and Iran's Nuclear Program would be desamated, if no completely destroyed.

If the attack is on the United States or Israel, there is only one way to put it. Millions will die, because that is what the goal would be. While an attack an Iran will most certainly be more of a strategic strike intended to cripple facilities rather than population.

Iran will be the one to use them.Israel is considering an attack on their nuclear facilities but even if they(Israel) had a nuke it may not do any good.It would kill citizens but might not even damage Iran's enrichment sites. The real question is has Iran already enriched enough to arm even one warhead.

Who keeps threatening to "wipe Isreal off the map"? Oh I forgot Iran is just interested in "peaceful" nuclear energy. If you believe that ,then I have some discounted Rolex watches for sale. For you(because I like you) I'll cut you you a great deal. If you don't like those then I can offer you a bridge at a great price.

2) US or Israel

I suspect Bush will launch a nuclear attack from a ship off Iran.

That's probably why he placed an ADMIRAL in charge of GROUND troops in Iraq.
