does it me i have to check in before the designated time and check out before the designated time? ex. check in-11am check-out 3pm

If your check-in time is at 11 AM, then that will be when your room will be ready. You can check into a room after that time. Sometimes, a room will be ready before check-in so you can always ask the counter.

When checking out, you have to leave before the time designated. If your check-out time is 3 PM, you have to be packed and out of the room by that time.

Check out is when they want you out by. Usually it is 11am, noon or 1PM. (with that 1PM being generous and more rare.) Some hotels start calling your room right at check out and start asking if you are planning to stay another night. Ruder ones just say they can charge you an extra night.

check in is when you get your room. A check in time given means don't expect your room before that time. A 3 PM time is usually given for that and Vegas is one of the worst for it. If you are earlier and the hotel has rooms ready they will give it to you. But it is generally saying that you can't show up at 6am and expect a room. Since most people are in transit, they don't check in until evening, so it usually isn't a problem.

If your schedule falls outside what they say you can request an early check in or a late check out.

No, it means that the earliest time you can try to access your room (via checking in) is often 12 noon and you MUST be out of the room by 3pm. You can request early check-in and late check out, but you almost always pay a fee and that's only if the hotel can accomodate your request.
