A life jacket is an inflatable vest that you wear when you are on a boat. If you fall into the water, it helps you float so that you don't drown.

Literally it refers to the thing you have to wear in a boat.
There probably are some cases where it is not meant literally and has some other meaning related to the first.
There is an expression, many people are in the same boat or lifeboat. That just means that all have the same situation and the same risk.

http://en.wikipedia.org/wiki/Life_jacket - pictures and an explanation. Basically, it's something you wear to keep yourself from drowning if you fall off a boat when you're far enough out on the water that you can't easily swim back to shore.
