Of course it is just acting. It is not an X rated show. In fact when sex scenes are filmed for TV or movies it is very mechanical. There are tons of cameras and people there, including a director who is yelling stop and start a bunch of times. In fact, from what I have heard from interviews with actors who talk about the "steamy" scenes they have filmed, the majority of them say that it is a bit awkward.

and no, the sex scenes are completely staged,
but the actors are actually performing the surgeries. I hear its a great way to get discount plastic surgery, kinda like getting your hair cut at a beauty school.

it's called acting and they're not nude. the guys wear a pouch with clear straps around their waist. it's a camera trick that they appear nude.
