I only have their names, and phone numbers. Is it really nessesery to write their address on resume? One of my reference is a co-worker in a fast-food chain. Another is a teacher.

NAME
PROFFESSION

is this okay?
What will I write in the proffession section on my co-worker's fast-food work? Do I write "co-worker"? Or "NameOFStore co-worker"?

1. Use spell-checker. You misspelled profession.
2. Include the name, title, store, address and phone number.
3. Profession should be the person's title. Your co-worker is not as good a reference as your supervisor.

I recently found this website. It is pretty good.
