It is 'blown out'. Or, I blew the lightbulb out. Or the lightbulb blew out. At least this is the way it's always been said in my family and friends!

It's gone to light bulb heaven. However, if it was a bad light bulb, then it's gone to light bulb hell. There is no purgatory for light bulbs, even the Catholic ones. :-)
