For decades, people disputing the theory of evolution asked, "where's the transitional fossil between a fish and a four-legged land creature?" The absence of such fossils was touted as demonstration of failure of theory of evolution. Recently, researchers have found such a fossil. See link about the Tiktaalik.
Of course, this won't deter detractors of the theory of evolution. They know, for example, evolutionists say that 2-legged hominids came from 4-legged creatures, so they're going to demand to see a fossil of a THREE legged creature, or else the theory is just bunk.

You cannot confirm a theory.
Also, the occurence of biological evolution is fact, not theory. The method by which it occurs is the theory. Anyone who tells you otherwise does not understand biology, even if they have a PhD (in anything, including biology).
To your question, though: one of the most complete examples of macro-evolution in the fossil record is the evolution of the whale and the evolution of the modern horse. You should be able to find charts/pictures detailing these in the links provided below.

Yes, of course. But you do not need any fossil evidence whatever to confirm it, as it can be proved simply by observing that the two requirements for it are both met: that variation occurs, and that natural selection occurs. Both of these are observed, so the theory is proved.
