I have seen a number of 5 pitch innings, especially by power 'closers', but never in all my years, witnessed a 3 pitch inning.

its a cardinal rule dat ur team isnt supposed to make 3 outs on 3 pitches.

I only remember seeing one. i think it was in 1988. The Twins were the ones batting, I have no idea who was pitching. I just remember John Moses of the Twins was the third batter and he popped up. The announcers made a big deal about how he should have taken a pitch.
i would bet it only happens once every other year or so... good thought provoking question!

I have maybe seen one. It is highly unusual. I have seen a few 9 pitch 3K innings by Johan Santana though which is pretty impressive.
