There are probably a hundred answers to this question. Without knowing the Millionaire whom you are speaking of, then no one can say for sure.
Some people who become millionaries get tired of all the bull that comes with having so much money, with people wanting things from you, yet not valuing you as a real friend.
Other have lived in poverty so long, that when they finally get money they fear going back to being poor, so they hord the money, saving it for when bad times come.
Others because of various types of mental disorders and or illness choose to live like a poor miser, as you call it, because the life style, the pressures of being a Millionaire are more then they can cope with.
Some people who are Millionaires have lived one life so long, before becoming well todo, they don't change their life style at all once they have more money.
You should ask him about his life, what was life in the past like for him. What are his happy times he remember? What about family? If he could do anything he wanted with his life, what would he change? Things like that, get to know him as a person and he may in time trust you enough to tell you the answer to your question.
Try being a real friend to him; that's probably what he needs more than anything else, someone who likes him and not his money.

Elderly rich people live with the fear of one day once again having to go without, hence the fact that They refuse to buy things that are not necessary. Ever heard of the "Depression"? Millions of people across the world lost every thing virtually over night as the result of the "Wall Street Crash". How would You like to have been in Their shoes (if They still had any)? Try to consider Their position before You cast aspersions.

Yeah, some of their ability to get millionaire do rely on their ability to save money and look for cheap "opportunities"... They just think they will wait until one goal that they keep raising until they die, therefore they will never quit saving money to reach the ultimate ever-raising goal that in reality is impossible to reach... Ambition is hard to overcome and more when you have the ability to reach whatever goal you set.... They have strong will and determination that end up working against them...
But not all rich people are like this though....

greed and a lack of intelligence. They think they will somehow take all of their wealth with them to heaven. But no one takes anything with them when they die not even their body or intelligence. We are eternal spirit souls, Part and parcel of the Supreme Soul also known as Krishna, Allah, Jehovah, Vishnu, etc. If one does not use their money in the service of God then they are a miser. Read Bhagavad Gita as it is By Bhaktivedanta Prabhupad which gives one the purpose and meaning of life.
