take a break, like 15 - 30 minutes
go eat some food, talk to people and stuff
then go back to studying.
then, every so often, probaly every 1 - 1.5 hours, take another break

You have to find a way to concentrate. The only way to remember things properly is to engage with the material constantly, so it goes to your long term memory storage. Simply passively studying or glancing over notes or being unable to concentrate will end up having you forget most of the material.
I would suggest trying different things like, making multiple choice questions yourself from the text maybe. Maybe study with a good friend, and quiz eachother, making it like a game you can engage in. This is very effective. If you have a partner it is also a good idea to use flash cards and make problems and quiz eachother. You can do this yourself, usually by talking to someone about the material later, say your parents. This helps you remember it well. You can also talk outloud while studying, verbally asking questions so you're more engaged with the material, and often going over things verbally reinforces the point better.

Ask the teacher for a blank copy of your previous tests and take them again. Most likely your midterm will be the same material.

Alright i had this same problem too what i do is i set a goal for myself and when i reach my goal i reward myself with something like going shopping after i study. also only study in 20 minute intervals because any longer and you lose focis. after 20 min get active for five min or so
