What are their interests or hobbies? Do they have advanced Alzheimers? If not- go with their interests and hobbies. Maybe they still like to read or listen to music. If they like to read maybe they need large print books.
If they have Alzheimer's go with something they can enjoy 'in the moment' like scented lotion or candy.
People that age feel the cold a lot so a warm afghan or fleece blanket might be nice. How about a robe, slippers, or pajamas?
Best yet- how about the gift of time- a good, long visit? Bring a photo album and reminisce.

My Nan recently had her 90th and we all were perplexed. I made her a family tree and framed it with a gold frame and card mount (from K-mart!!!). I used those scrapbook type papers and made a simple cream background, cut out a tree shape and punched out heaps of little cream and mint coloured leaves, a bit of ground with heaps of punched out cream and white flowers. I printed all the names of our family on white paper on the computer and cut them out in little leaf shapes. I glued them on the tree and glued (with white pva craft glue) tiny bows made from really thin ribbon (bought at Lincraft) at each leaf. She LOVED it and even though her eyesight is poor, she shows everyone her family with pride and joy. If your gift is for a man, just use blue ribbons or chocolate colour. It does take a bit of work, but it looks sensational. She also received - a crystal napkin holder, a photo/scrapbooked album, a beautiful soft lambswool throw rug, roses, a stunning silver letter opener. Hope this helps you out?

I would say a nice warm quilt. Older people can get awfully cold sometimes because of poor circulations.
