I am taking temporary custody of my nephew. Legally I am filling out a caregiver authorization affidavit so that I can get him inrolled in school. I need to know if the parent (my brother) needs to fill out any form(s) to give me the authorization to become a caregiver. He is not giving me legal custody of him. Please advise. Any websites that spell out the procedure would be helpful. THANKS!!

I really tried to find an answer to this question for you (and I am pretty good at such things), so I can understand your difficulty in getting an answer. My impression (but it is only that) is that you need documentation as a caregiver only if you are trying to receive benefits or assistance based upon that status. If your nephew is resident with you, even if you were not an authorized caregiver, I believe he would be entitled to enroll in the local school. (The effect of your being a caregiver would be that he could stay enrolled at his old school.) But I am not confident in this answer.

My only suggestion is to call two agencies. First, the registrar at the school or school district, to find out what documentation, if any, is needed for enrollment. Second, your local social services agency to see what documentation, if any, is needed for you to act as a caregiver in your home.

purely call your courthouse the next day and ask who to speak to approximately constrained guardianship - the place you will get scientific coverage, carry the youngster to the scientific expert and join the youngster in college. additionally, I googled "Caregiver Affadavit KY" and have been given a number of hits which have been for KY varieties.
