Two more great ones that haven't been mentioned yet are:

Dominick and Eugene
Plot summary from imdb:
Dominick and Eugene are twins, but Dominick is a little bit slow due do an accident in his youth. They live together, with Dominick working as garbage man to put Eugene through medical school. Their relationship becomes strained when Eugene must decide between his devotion to his brother, or his need to go away to complete his training. Things are also not helped by Dominick's co-worker, or Eugene's budding romance.

Charly
Plot summary from imdb:
Charly is a retarded adult male struggling to survive in the world. His frequent attempts at learning, reading, and writing prove worthless. His teacher, Miss Kinian, takes Charly to the clinic where he is observed by doctors who have Charly "race" a mouse, Algernon. Algernon, a once retarded mouse, is usually the winner thanks to an experiment that greatly raised his intelligence. This experiment is given to Charly, who at first does not seem affected. However, he gradually becomes more and more educated, eventually becoming a pure genius. Very emotional consequences are involved when Charly learns the truth of the experiment, however.

Spider - directed by David Cronenberg starring Ralph Fiennes, Miranda Richardson and Gabriel Byrne. Mental illness beginning in childhood has grave consequences. Film slowly unravels into Oedipal tragedy but the way its directed puts us in the place of the ill boy/man Spider (Fiennes) and shifts the viewers perceptions to that of the mentally ill hero.

Its set in 1940's (?) London a time when schizophrenia or other mental illness was only diagnosed as madness. A dark but brilliant film.

The ones with the best performances in my opinion, are Rain Man (Dustin Hoffman), What's Eating Gilbert Grape (Leonardo DiCaprio), and Dead Man's Shoes, which is a small independent British film directed by Shane Meadows.

None of these actors manage to patronise the disabled characters that they are playing, which is vital when playing one of these roles.

Rain Man, I think it was a great film with Tom Cruise and Dustin Hoffman. Hoffmans' character suffered from autism, I think, but it was directed very tastefully, and both actors were teriffic, one of Cruises' best performances.
