Go to your local post office.

They will have the applications. You will need your birth certificate and passport photos as well.

The cost is up to $95 I believe.

Call your local post office; many post offices process passport applications but not all of them. If your local office doesn't offer this service, they'll tell you which one does. They will set an appointment for you to come in with your fees, pictures and documents. You can't just walk in--- you must have an appointment.

You can go to your local US Post Office to get you passport. If you go the USPS website they will have all of the information you need, as well as you can print out the forms beforehand.
