Well you could do a scale of motivation, how easy if it for you to get out of bed in ghe moring, very easy, fairly easy niether, hard very hard. etc or use a number scale.
Think about questions such as how many days sick have you taken from school or work. When are you most likely to complete a form or assingment e.g or several weeks before, the day b4 etc.
Do you want a promotion, who you be interested, how would you get the promotion etc.

What do you like about your job?
What don't you like about your job?
Explain what a 'good day' at work would be like.
Maybe a multiple answer question like....If you wake up not feeling well and contemplating calling out sick what would change your mind to choose to go to work anyway? a.) no go to work, no pay b.) will fall behind too far and don't want to be over loaded when you return to work c.) You have a crush on a coworker d.) don't have any sick time etc......
I'm not sure what kind of job you do but if you could come up with any fictional senarios to make like a 'What would you do if...' type of questions that may help.
Good Luck.
