Several things at once. Comparing the appearance with what you anticipated. Comparing the appearance with what is acceptable. Looking for body language and watching your own. Once you have done your 15 second look your mind becomes judgemental. It's up to you then to believe that 1st impressions count or that it's what the person has to say that matters.

You are judging a book by its cover, that is you are making a decision about someone before you know enough about them to make a decision that means something.
Another way of looking at it is you are being prejudiced. Say you decide you don't like redheads because of the colour of their hair; it's not about the person, it's about how you pigeonhole people without thinking about it.

Depends on what judgment you make. If you judge his appearance, like 'pretty', 'ugly', 'weird' ... then you are acting quite normal. Everybody does.
If you judge his/her value as a person than you are not only likely to be mistaken but worse : the legal term is : discrimination.
