No there are still many who use CDMA. TDMA is starting to disappear in favour for GSM, and now that Sprint owns Nextel, it's possible that they could swallow up the iDEN network in favour for their CDMA.

The carriers who use CDMA would be Verizon (the largest), Sprint PCS, Cricket, Revol, Alltel, and most other former wireline carriers i.e. Centennial Wireless, etc. There are other carriers who buy airtime in bulk from these carriers but provide their own service (also known as MVNO's) like Virgin Mobile.

It should be noted that Sprint PCS, Cricket and Revol are PCS carriers. This means they still use CDMA but primarily on the new(er) 1.9GHz band. Also, Sprint and Revol will not activate phones from other carriers, even if they are technically compatible with their network. Sprint, Revol and possibly Cricket put locks on their phones to prevent other carriers like Verizon from activating their phones.

CDMA carriers - Alltel, Amp'd, Cricket, Helio, US Cellular (in some markets) Sprint, & Verizon are the major users of that Technology.

As mentioned above there are several smaller or Local Carriers who use the technolgy as well.

heck no, verizon is still cdma, sprint is technically cdma but on a different band, a lot of small carriers are still on CDMA. Among large carriers only Cingular and TMO are GSM, not counting MVNO resellers.

An unlocked CDMA phone would be what you are looking for.
