If they like their shirts tight then a small...a more casual look medium.. i like to go with medium if it's 100% cotton because they shrink a little when you wash em

It depends on what their sense of style is.
Small if they are going for that boy band chic. Or medium if they have more of a sporty sense of style. Make sure if you get the small that it looks extended in the torso...Express for men has nice t-shirts. But Hollister and A&F I've noticed have similar fitting clothing.

It really depends on the width of your shoulders, length of torso, etc...
And those brands run so small.
I recommend you check out the mens line at Breezy Beach Wear. They have the same beach bum, surfer guy style, but their shirts fit better and they are typically less expensive. They offer neckties and hats as well.
