I'm 21 and for my birthday I got a lot of alcohol-themed stuff such as shot glasses, wine, etc. You could also take her to a nice club and buy her lots of drinks! lol
If she's not into that, treat her to a massage, manicure/pedicure, facial, or something of that nature. Girls love to be pampered but rarely take the time or money to do it for themselves.
If you're just looking for something small, it's hard to go wrong with candles, picture frames, favorite lotions, etc. or a cute piece of jewelry--earrings, necklace or bracelet.
If there's a movie you saw together that she really liked, that would be nice, or concert tickets to see her favorite band. If she's more intellectual, get a book that you really like or one she's been wanting.
I'm sure she'll love anything that's thoughtful and shows that you took time to pick out...it's much nicer than gift cards sometimes.

The best would be buying her all of her drinks and setting up a huge party for her. The most unique would be to get her something she likes. If she has a hobby, figure out what it is and buy her stuff to do. People always like it when you have to put thought into a gift.

A gift basket of alcohol... 21 nips of different types of alcohol. If you really want to spruce it up, make the basket an ice bucket, throw in a shaker, and some mixes to 'accessorize.'
If you're going a cleaner route for someone who doesn't drink, I would definitely look on gifts.com for some great ideas. It'll definitely point you in the right direction.

Actually there is some merit in the alcohol themed gift basket. unless she is an alcoholic she is unlikely to acquire some handy items on her own, and this is the only birthday when people with think to give them. Shot glasses aren't all that cheap, same goes for bar sets (shaker strainer etc.).

My favourite 21st birthday present was from one of my best male friends... He got me a spa day... complete with Facial, manicure, pedicure, and full body aromatherapy massage. If you can't afford all of them, just the massage will do.... I don't know of anyone who doesn't appreciate a good massage.
