The best way to completely and safely destroy a disk is to run it through a cross-cut or diamond-cut shredder. The company that I work for has several of these that we use to destroy sensitive data as well as out-of-date back-up disks. You can pick up a good shredder that can destroy CDs for under $60. If you are destroying a lot of paper or disks this would be a very good investment. Any of the companies that I listed below are a great place to get a shredder from.

Destroy the side where the metal is. That is the surface that carries the data. Not the plastic surface which does not matter. Break the CD. Split it with an ax. Scratch it. Any way that ruins the metal surface is good.

The most often method I have seen senior techs use here is to user a tool to cut the edge of the CD, cut about one inch in. It seems to affect the TOC for the CD

Most newer paper shredders on the market can shred CDs as well. Which doesn't help you at all if you don't have a newer shredder.
I usually just break them, but you have to be carefull with the splintering plastic.

Use a piece of cotton dipped in acetone to and wipe the surface with it. If you are lucky, the silver stuff wil dissolve. If not, the plastic becomes soft and you can scratch it all you like.

Break it, scratch it beyond recognition, it doesn't take much to make CD's unreadable.
Threes also shredders that shred CD's and credit cards.
