Internal organs are damaged by impact (bruising) which may lead to rupture, toxins (either ingested or breated in), or oxygen starvation. Malnutrition in the longer term can also damage organs because it deprives them of the proteins, fuels (fats and sugars) and micronutrients (vitamins and minarals) required for protein synthesis, metabolism and growth.

Smoking - lungs
Alcohol & drugs - everything as under these to quote my late greatgrandmother "Drinks in wits out!" hence leads to all manner of damage, either immediately physical, longterm physical, or via spread of any number of tranmisible diseases.
