The pressure has kept getting higher until now it pegs out the low pressure gauge and is venting through pop-off valve.
Even with the adjusting screw all the way out.

Can I disassemble and clean something to get it working right again?
Is there anything that is safe to lubricate the "innards" on a oxygen regulator?

Take it to your welding gas supplier for repair. Replace the regulator immediately.

Do not lubricate an oxygen regulator!!! Ever!!!!! Oil under pressure in oxygen will explode. No ifs, ands or buts. It will happen. Don't blow your hand off. Replace the regulator until its repaired.

"Inspect the regulator and cylinder valves for grease, oil, dirt, and solvent. Never use
grease or oil to lubricate regulators or cylinder valves because they can cause an
explosion."

it is faulty, take it in for repair or replacement, you CAN lubricate them, but MUST use something such as graphite. NEVER any oil based product whatsoever. sounds to me like the diaphram is busted, or the spring. maybe both. basically, it is trash unless you replace the insides.
