Grammar Help, when to use have, do, various forms of verb be?

Please refer me to a website or book that will explain when to use does/did, do/did, has/had, have/had. Thanks.

Most of these do not show the entire conjugation ... only the present/past/future tenses.

Ask your teacher, local librarian**** for help!
