How do you punctuate things in quotes?
For example:
"Please, Mommy, Please!,” I shrieked.
Do I need both an exclamation mark and a comma?
How about this one?:
“And Kayla?,” she asked.
botha comma and a question mark?

No comma after the exclamation mark. No comma after the question mark. :)

Those are used only in instances where the sentence is cut in the middle by the phrase, like this:

"I called," said the girl, "but my mother didn't answer."

See?

Note that I got rid of the commas and that basically each sentence was split into 2 sentences. So the S in She is capitalized

I agree with the other 2 answers, no comma is needed when the sentance has punctuation in place.

As a general rule -'speacially when yo' find yourself.. "tossing-a-Peso" , (over "The Desicion")-

is to place the comma, EARLY; and (often), now-a-days: MISS OUT the second comma -altogether!

It works, fine!
-Always, AND looks.. 'modern'.

need only an ex'n mark, as it acts like a full stop, and finishes the sentence.

“And Kayla?” she asked.
same as above.
