Because marriage is a legal issue. In the United States legal issues are determined by the courts. Simple as that.

Lots of reasons including; one party still married, fraud committed to marry, party not legally authorized to get married, party didnt meet state requirements, party underage, married under duress/forced marriage, etc. Usually voided as a result of a violation of the law

Usually a marriage under false circumstances (immigration) duress, or a technically such as one spouse already being married

If the two people involved decided that they could NOT live as a couple,or if one of them was still married to someone else. Or if one of them was underaged(wow that is a rough one) or if one of them was drunk and didn't realize what they were doing.
