Go to http://www.plannedparenthood.org to find a local Planned Parenthood, check out http://www.4woman.gov for resources and various programs in your area, and call your local hospital (speak with the Patient Advocate there) or women's health clinic (check the yellow pages) to find low or no cost medical resources. They will be able to suggest various programs that might be in place already and/or direct you to the nearest medical resources for well-woman or basic medical care.
