I went to the website http://abc.go.com/primetime/extrememakeover/show.html to find out info and to sign up.. but damn they want to know a lot of personal shi% man.. what is with these people.anyway has anyone else applied? I will fill it out

They ask the questions because getting an extreme makeover is a very serious matter. They need to know that the contestant is mentally stable, and has proper reasoning to go through with surgery, because it is life altering, and although it may fix some physical and even some psychological issues, there are some matters that a makeover will not help. Plus they need to have a good story and personality so they can relate to the audience.

Since there is more than a million bucks involved these questions are suppose to cut down on fraud.

they want to make sure you are someone the audience will connect with, someone people will think "deserve" an extreme makeover which will increase ratings
