Deception, treachery, mental illness and murder are all effects of isolation in Joseph Conrad's "Heart of Darkness". The hiding of one's true feelings and desires is manifested by the isolation; that isolation, itself, may occur in any place, at any time. It is the perspective of those who would chose it, or have it chose for them. The result of which, springs forth doubt and incontentance among the many who might falter along this path. The treachery of isolation is that one's mind begins to fold in upon itself ever increasingly unable to decipher fact from phalicacy, creating an ever absorbant abnormality to the cerebellum. Torn between the id and the idiot, so to speak, we can see that Conrad is making clear the dementia that follows when one isolates one's self from the outside stimulus of reality.
This leads to mental fatigue and stress fractures along the senaptic pathways of the glutius maximus, rendering it's victim forever lost in a sea of what they percieve to be meaningless babel. As this dementia progresses, a murderous victim may, indeed, feel that they are protecting themselves while, in fact, they are committing acts of mass murder. One might draw a parallell from Conrad's work to accounts of Jack the Ripper, and other nortorious derranged characters from literature.
