That depends on the kind of person the boss is. Personally, I couldn't get rid of someone like the woman you wrote about fast enough. But, some bosses see employees like her as an asset, since she's a built-in spy and a$$ kisser.
One thing to consider, if she insults other employees to their faces and is still employed there, she's got some kind of protection thing going on. I worked with a woman like that several years back. She was the most vile person I've ever worked with. Back-stabber, world class gossiper, brown-noser and she was the most envious person I had ever known, all wrapped up in this "I just want to be your friend" package. The one good thing about being downsized (and it was devastating) from the company was at least I didn't have to work with that vile person anymore.
I would say that if you feel you have any value to your company at all, (and you obviously do, because you've stood up to her before) I would stand my ground with this woman. Depending on how much help you feel your company HR department is in situations like this, I would document the actions of this woman. Also, if you feel like you do about her, others in the department probably do as well. I would seek their input. Perhaps you all can join together in voicing your professional displeasure about the office behavior of this woman to the powers that be.
Regardless, I'm a great believer in karma. And, there's no karma quite as just as office karma. What goes around, comes around. Rest assured, one day, this woman will get hers. Before you know it, you'll be singing "...ding dong, the witch is dead..." and working with this woman will be a thing of the past for you. Just keep taking the high road and you'll come out on top.

I wasn't exactly "sabotaged" at my last job (I now work at home and love it!), but there were a couple of people, 2 women in particular, (one was the office manager) that were the "office instigators." They would talk about the other employees behind their backs; they even talked smack about each other to the other employees! I am so glad I own my own business and work at home - I don't have to put up with backstabbers anymore. If the trouble maker is NOT the office manager or a supervisor, then she has no business making a log of what everyone else is doing. There could be a number of reasons why the boss does not let her go, reasons that you may not even be aware of. We had one such person where I used to work, and when we complained, we were told by our boss (who was also the owner of the company) that they were well-aware of the problems this person caused, but they had been advised by their attorney not to fire her. Or it could just be poor management - a manager that just doesn't want to deal with the issues. If you have someone in your office that is a backstabber and is talking about other employees behind their back, then it is possible that you could be sabotaged at work. That person is probably talking about you/keeping a log of your activities behind your back as well.

Check with HR. There should be some policy about spying on employees. Also make sure you copy any files on your computer. I took a day off 2 wks ago, came back and my whole file was gone, along with 1 completed project and 1 almost completed project. I now have to redo my projects and countless other guides and spreadsheets. I hate shared files!
If you wanna have some harmless fun, here's a prank I saw in Maxim a few years ago: Unscrew back of her chair, slide raw shrimp into support pole. Reattach back. No one will ever figure out where the stink is coming from.

Welcome to the "real" world of business. That's how all businesses are run. The brown nosers & backstabbers that do no work get ahead whereas the good workers get nothing. The boss won't do anything about it. He likes having his butt kissed. Why ? It's all about his pea-brained ego. At home his old lady wears the pants in the family. The only time he can be a "real" man is at work. nickle has another point I forgot too.

Just do your job. Have a good work ethic and high standards.
There is nothing to worry about, unless you are behaving inappropriately.
Keeping a log of other employees behaviour makes her look silly and demonstrates a lack of seriousness about her job. A good employee has more productive things to do. It's very difficult to "let someone go" without valid reason.
If you make an issue of it at work, then you might get "let go" because of interpersonal relationship issues.
Just do your job.

Well, there goes a saying of "Misery loves company." or "What goes around comes around." Let her have her spiteful fits and you continue to do your job. Trust me, the boss notices what is going but pretends not so. She thinks her ignorance and rudeness will get her far but if anything it will be a set back. Nobody likes an imposer, and that's what she is. God doesn't like ugly. Pray and let him handle the rest. I know, I've been there and done that.

just assume you are, most of my jobs had at least a few people like that around
a good boss would see it and would just fire the kiss as s people and
would reward people for what they really do, not fake smiles and bitchin about others

I strongly suggest you report this to HR, if not, you'll end up being let go, if she has it her way. Don't wait.
