In regards to health care...Who provides the care? Is the focus on self-care , or care provided by a family member or another? And please list your country. Thanks

USA care is based on your wealth and the type of insurance you have.

If you have no insurance and no income then you will get limited health care. You can visit the free clinic. You can also go to the state funded hospitals for emergency care. But the will hound you to pay your bill until they eventually write it off.

If you have a job with a poor health plan then you will get poor health care.

If your job offers a good health plan you will get excellent health care.

And if you tell you job you have a major illness (which this is illegal) they will not hire you.

In Malta:

government provides completely free health care inclusing all surgeries except those for cosmetic purposes to all citizens , ppl who have been paying taxes for more than 6 years, the british (as ther is some aggremment) and sanctions other embers of the EU.

a lot of medicine for long term diseases is also free. the elderly get a monthly check of around 25E to spend on additional
medicine

there are also private health insurances but they havent catched up well yet and the majority who are covered are only covered because as it is an infringement benefit which comes along with their job
