You are correct. The ball is next spotted on the yard line from where the kick was attempted. You'll see the referee put a bean bag on the yard line the holder is on when he sets up. If this point is inside the 20 yard line, it goes back to the 20. Your senario is interesting because (obviously) it's never been attempted. The longest field goal attempt was 69 yards, but that was on a free kick. I beleive that it would be ruled a safety rather than a touchdown. That is because a missed field goal is technical a violation of the rules in the same way an incompleted pass is. The violation occured in their own end zone and causes the ball to be returned there. Also, a touchdown requires a player to be in posession of a live ball in the opponent's end zone. In this case, the opposing team is not yet in team posession of the ball, let alone player posession. Thus, everything points to "safety." The team that missed the field goal would have to free kick the ball again from their 20.

The field goal ball is the exact same shape, size, and material as a regular game ball. However, most kickers will want to use a new or newer ball which will be more stiff and harder to hang onto while running.

They use completely new footballs for most kick plays. For normal plays, they rotate balls that are new that day, but not new for that play (other then the first plays of a game).

Nope, the kicker can choose the ball, but it must be licensed by the NFL.
