Istead of just bulllet pointing what you observed you're sort of writing a little narrative of what you observed.

For a pre-schooler reading, you will probably be looking for literacy behaviors. Be as descriptive and objective as possible to explain what the child CAN do. (Don't worry about what the child cannot do.)
Examples:
-opened book correctly
-pointed to text, made up words
-created a story to match the pictures
-moved finger from right to left while pretending to read

Look for certain key behaviors, like:
-directionality: does the child know which way text should go?
-print: does the child know that print holds a message?
-book handling: does the child know how to hold a book? Turn pages?
-story elements: does the child improvise a story that matches pictures in the book? mention key characters? setting?

So, to put it all together, here is one example.

Kayla is a reader at the early emergent phase. She approaches the reading task with excitement and enthusiasm. When she first opened the book, she held it upside down. She noticed her mistake when she saw the elephant picture, laughed, and reversed the book. She pointed to the title and said, "The Elephant's Day"...etc, etc, etc.

an annecdotal is simply a narrative account of what you observed...EX: "today in class Johnny was very active during reading time. he would not sit down nor be quiet even though the teacher asked him to do so 3 times. he acted out when dealing with other students"

OR
10:00: johnny was out of his seat

10:10 teacher made 3rd request for him to sit down, he ignored her.

10:15 johnny was sent to the principal's office for failing to obey rules.
