If you are talking about hardest NATURAL substance, it is diamond, without any question, with 10 hardness in moho's scale.
But some man-made substances like Beta Carbon Nitride (β-C3N4), Fullerites, or Polymerized Single Walled NanoTubules (P-SWNT), (1.13 times harder than diamond), Ultrahard Fullerite C60 (1.17-1.52 times harder) are also experimentally prepared.

I believe diamonds are the hardest substance in the world. Rocks are tested with diamonds at the end of the hardness scale.

Recently a substance harder than diamond has been synthesised, it is called ACN, which stands for Aggregated Carbon Nanotubes (some slight variants on the name exist). It scratches diamond with ease. Check out:

I would agree with a diamond.....
However, if you asked what the most dense substance in the world is....my wife would say my head.

Diamond. A 10 on the Mohn's hardness scale.
There is some dispute that carbon nanorods are even harder.
