If you have an agent, you use the agent's contact information. That his/her job. And if you don't include that, it looks like you're trying to cut your agent out of the loop. Your agent's job is to handle that for you. They weed out the useless calls, and handle any negotiations, usually better than you can.
If you don't have an agent, use your own address; you're effectively your own agent. If this is a resume to an agent, include all of your usual contact info: phone number, address, and email.
A lot of actors are creating personal web sites, both as an online resume and to handle their email. I think it looks snappy; Hotmail or GMail addresses always look too personal.

Haven't you ever seen one of those sitcoms where the guy puts the number of the pay phone on the corner and is continually having to kick everyone off so he can get the call of his dreams? Well, you don't have to do that but I used to put my home phone on my resume all the time....especially if you chose not to have representation.
And I guess I should add that I was in show business for many years.

I'm not an actor, but I'm a theater professional. My resume has my name, address, phone number and email address. If they're going to contact you based on that, you want them to be able to actually get a hold of you.

Your cellphone number is sufficient. You don't want to use your home telelphone number as you never know who will have access to your resume. Your address is not necessary.

My resume has my home address on it; I've never really given it a second thought. I list my cell phone #, and my e-mail address.
And that's it. If they want to find me, they'll find me.

NAME
ADDRESS
CONTACT NUMBER.. (where you are able to be reached at all times, like a cell phone0
