You don't have to follow any exact form... just write something up that states you are leaving in 30 days (or more) from today also put the date of your last day.

State that the apt./house will be clean and ready for inspection on a date a couple of days before the 30th day (so they can see that you have cleaned) and then state that you expect your full deposit back within 30 days of your move out date (they really have 60 to give it back, legally, but most don't know that).

Give your forwarding address if possible for them to mail the check.

Sign it and get a witness (anybody can be a witness) to sign it too... then send the original (keep a copy) certified mail with a return receipt so you can show they did receive it.

Then sign it, copy it and send it. By the way, not a bad idea to send it certified return receipt requested. Some of these landlords are jerks.
