The nail was put there when the tree was much smaller. Over time the tree has gotten bigger and grew around the nail.

Most likely the nail was put into the tree at an earlier time and the bark grew over it and covered it up.

A nail was nailed into the tree when the tree was younger, the tree then grew around the nail. Happens all the time.

The tree grows from the inside out. The nail was driven into the tree, then the tree grew around it. It also happens with wire fences.

The Tree basically grows around the nail this is called inclusion.. as it the same way a tree will loose a branch and cover the wound. This process is called Compart-mentalis-ation and is how the tree protects its self from infections.
