Yes - sometimes 1 ball doesn't descend like it should. He should see a urologist because undescended testicles can turn cancerous.

Later cancer risk

One of the strongest arguments for early orchiopexy is prevention of testicular cancer. About 1 in 500 men born with one or both testes undescended develops testicular cancer, roughly a 4- to 40-fold increased risk. The peak incidence occurs in the 3rd and 4th decades of life. The risk is higher for intra-abdominal testes and somewhat lower for inguinal testes, but even the normally descended testis of a man whose other testis was undescended has about a 20% higher cancer risk than those of other men.

This is not an uncommon congenital condition and it is known medically as UNDECENDED TESTIS. the one that is missing has been stopped in its path in the abdomen ,pelvis or groin.

This will not affect his masculinity or his ability to make a woman pregnant. You will have noticed (if you are experienced) that nature will have done a Compensatory Hypertrophy (bigger than normal).

The only problem is that he needs to be seen by a Urologist and have the undecended one removed as it is more likely to develop malignancy than a normally decended one.

What about it? It happens sometimes. I dated a guy with only one, and that was weird for me, seeing something like that for the first time. Everything still works the same, though. It's just an oddity that happens once in a while.

It is definitely possible that he was born that way. The testes descend and sometimes the vas deferens become entangled and they choke off a healthy teste, leaving the person with only one healthy teste.

He probably has the second one in there, it is just so small and probably just hasn't descended.

This is a serious answer, unlike many others you are likely to get.

There is a possiblility that one of his testicles could be trapped inside of him a little higher than the pelvic region. I had this problem, and its fairly common, mine was diagnosed when I was 8, fortunately they got me when it was still possible to operate.

Maybe your friend wasnt so lucky.

Must of been like that when he was born. If he finds it abnormal or it embarreses him, you can get a "fake testicle" insert to go into the sac.

I don't know, but then there is a 50/50 chance, with one testicle, you may get pregnant then.

That's not a real problem one will suffice. I was born with a little oddity myself one of mine is just a little bit larger than the other two.

It happens. Not everyone is born "perfect." Your male friend can probably still father children and I'm sure everything "works" just fine.
