Ideally, everyone will monitor themselves.. but you get people of all maturity levels there and some people abuse the trust you give them.
The first rule, and the one that resolves all problems: Communicate.
Ask if you're not sure whether you should Need or Greed it. People will give you their opinions. If two people Need it, then everyone else can pass on it and those two people can roll for it.
After you've talked about it (if necessary), what works for us:
1. Make sure your loot settings (chosen by leader) are the way you like them, from the beginning.
2. If you don't Need an item (i.e., have something similar or simply can't use it), pick Greed automatically. Basically, if you're planning to sell it for cash, that's Greed; and everyone should get a fair shot at winning the item.
3. If you can use the item, ask the group if you can "Need" on it. Usually they say yes, if no one else Needs it -- or at least anyone else who "Needs" it can have a say as well.
4. If you find a chest that anyone can open, /roll for it manually, and then winner is allowed to open the chest.
5. If people don't follow these rules, even after having them explained, then the group can decide to kick the person... or at least never instance with them again. (Why instance with someone you can't trust?) If the leader won't enforce anything or your group is lame, then you can either bail or else finish off the mission and then not instance with the offenders again.
The guy who was wantonly looting everything is a guy I would have warned, then bumped from the group.
These rules are very important in regard to BOP items -- since once someone grabs the item, no one else can use it, even if they change their mind.
Usually people like to instance with friends, when possible; so they extend courtesy to each other. Also, people who find items and can't use them -- if they are friends -- often ask the rest of the group if someone needs it. It's very much a group outing, not an individual lootfest.
I admit, the last time I instanced, I had very very bad luck -- I think I only won 1 item on all of my rolls, and didn't actually Need anything that dropped. But that's just the way it goes, I guess.

There are literally dozens of 'loot rules' out there that diff groups use. Perhaps this will help, as it is a fairly common rule on the Earthen Ring server and one I think is fair for everyone.
1: Anyone can loot cash, or any non colored item (ie green+)
2: Greens are always Greed rolls, unless you choose to pass.
3: All Blues and Purples are passed by EVERYONE in group. Anyone who does not follow this rule gets one warning then kicked. No exceptions. After everyone has passed, those who need the item /roll for it first. If there is no one, then everyone can /roll for it.
4: Once a player in group has a Blue+ item, they are excluded from any other blues that drop until everyone has gotten a blue or better. Exceptions to this rule would be if, say, there was one Hunter in the group and 2 Beaststalker items dropped. Most times the group will automatically let the Hunter have the items.
It's all about being fair. BoE Blue+ items are considered AH items usually, unless someone can obviously use the item to better their character. Hope that helps clarify some things for you. Like otehrs said tho, if someone is grabbing everything in sight... kick them. One warning, then kick (sometimes no warning if they're just being... jerks.. to use a nice word :)

In my guild, if you absolutely need something, you may roll 'Need' without asking. However, if you don't absolutely need it (but rather want it), or is is something that won't directly enhance your class skills, then greed it. You don't need to pass on things you really don't need; instead, roll 'greed' on everything you dont need. If you win, you can either disenchant, vendor, or Auction House your loot.
In some cases, your group will establish looting protocols for the classes in it. For example, it may be established that a mage in the group gets the only need roll on robes or staves without healing bonuses, or the only warrior gets need on plate that drops. Follow these for courtesy. If there is an item you need, that is not designated for a member of your party, need it. However, I always ask 'May i Need?' if one of these items come up in a public group, and I am not sure as to how they would respond to a sudden 'need' when I may be expected to greed.
For telling what you may need, it may help to follow these guidelines:
1. Is the particular armor, weapon, or misc. item dropped better in stats etc. than the same type of item (shoulder armor, for example) that you're wearing?
2. Would it benefit your class' capabilities?
3. Is it greater than 5 levels of your current same-type gear piece?
4. Could you need it more than another member in your group with the same ability to use it?

Usually the way it works it if you need it and can wear it at the time, go ahead and need. However, if someone is able to wear it before you, it goes to them "if" they need it. Otherwise, if multiple ppl want it, you all roll "need".

if you need it, grab it. unless there is someone who keeps grabbing everything, try beating them to it, especially if you really need it. don't grab everything you don't need because you wouldn't want everyone else in your group will feel the same way you did when you experienced it.
