after i tried all the options you all gave me....and it says " Windows was unable to complete the format."...what must i do?

Try to format it again. But most probably that floppy is useless now. It might have got created a bad sector in it.
Floppies have never been reliable. you never know when will stop working.
Though there are some programs that can format the bad sectored floppy disks, but they are not 100% perfect.

Try formatting it again with another computer. If you can't, most probably it's damaged.
