a douche is a bag filled with liquid that a woman can use to cleanse her vagina. being called a douche is simply an arbitrary insult.

'douche' is french for 'shower', but used in english for women cleaning their nether regions. It was widely used in the 1970s as a really bad curse - like today you would call someone a MoFo, then douche was just as bad.
Funnily, I am watching Clerks 2 right now! haven't got to that part - funny movie :)

Creative insult meaning that you someone is a horrible person, probably racist, obviously crazy, and a complete jack*ss.

Its a creative insult
Basically calls someone a Nazi which can have any number of implications (they're an asshole, like to stereotype people, ruthless, german, very meticulous, or even just blonde lol).
The dousche bag part just means they're nasty, and unsavory.
Nazi dousche bag- person who is a nasty asshole.
